Applicants must meet all of the following criteria:

• U.S. citizen currently residing in the continental United States

• Established patient at a SART-accredited, board-certified fertility clinic

• Medically diagnosed with infertility by a licensed physician who will certify the diagnosis

• Maintains adequate living arrangements (rents or owns a residence)

• Has not yet completed the requested IVF or IUI cycle

• Grant can be applied toward future, not already-completed, treatment

• Non-refundable $50 application fee
